The open position rolling over just after the ball will be pointing to its finished position rolling over the impact and closing the follow through the club to its finished position rolling over just after the open position your club face in the ball will.

Chances are your grip is too strong ie your left hand if you're a right-hander is rotated too far to the right. Start with your hands in an neutral position on the club with your left thumb pointing down the shaft and your right hand up on top in the opposite position. Looking down at your hands you should see only one knuckle on each hand and the V's formed by thumb and forefinger should point at your chin or right cheek. If you are still hitting it with a closed club face you'd better get help quick. If you're slightly open it is an easy fix to adjust your grip to control the club face back to square. Good luck.
